Membership Fees
To become part of this new and exclusive medical practice, patients will pay an annual concierge fee.
The fee does NOT need to be paid upon your first visit. We ask that you schedule an initial consultation to ensure our practice is a good fit for you. Prior to joining our practice we ask that all patients read and understand the rules and regulations regarding the practice.
The concierge fee should be paid in full before your second visit. Health insurance does not cover the annual concierge fee. BCIM does accept personal checks, Visa, Discover, American Express or Master Card. Please call the office regarding pricing for uninsured patients.

Insurance Information
PPO Patients:
BCIM is an In-Network provider for Blue Cross Blue Shield, Humana, United Healthcare, Cigna and Tricare. BCIM accepts most other PPO insurance as an Out-of-Network provider. We, however, do not participate in Medicaid. Regardless of your plan, we will continue to bill your insurance for all covered services. It is our intention that no insurance-covered medical services are included in your annual fee.
Traditional Medicare Patients:
BCIM participates fully in the Medicare program. Please advise us if you have recently turned 65 when you sign on with the practice. It is our intention that no insurance-covered medical services are included in your annual fee.
HMO/Medicare HMO Patients:
HMO patients will not be able to join the practice until they transition to either a PPO or traditional Medicare plan.
